Privacy practices
In compliance with the Health Insurance Portability and Accountability Act (HIPAA), the notice of privacy practices has been posted below for you to review prior to your appointment.
Would you like a cost estimate?
At Acosta Physical Therapy, LLC, we believe in cost transparency and your right to shop for high quality, cost-effective services. We will be happy to provide you with a“Good Faith Estimate” of our costs upon request or upon scheduling. Call our office for more information or see our fee schedule posted below.
Treatment times range from 60-75 minutes. The rate for initial evaluation/treatment is $150 and subsequent treatments are $125. Payment is due at the date of service. We accept cash, check, or credit card. Please see our cancellation and returned check policies in the intake forms.
Physician referrals and insurance authorization are not required.
Acosta Physical Therapy, LLC, is out of network with all insurances and unenrolled with Medicare. Medicare beneficiaries will not be able to be reimbursed for our services here at this clinic. We do not bill insurance companies. However, please advise us if you plan on submitting an invoice to your insurance or Flexible Spending/Health Savings account. We will fill out the superbill with the appropriate information and provide it to you to independently submit to your insurance.
We do not accept workmen’s compensation, motor-vehicle accident, or other personal injury claims.
